A good strategy is described in the Hugin tutorials titled "Stitching Flat Scanned Images" and "Creating linear panoramas with Hugin".

The same strategy seems to work OK using newer versions of PTGui (version 4.1 or above). Older versions of PTGui had problems with large shifts (d/e) causing images to be cropped in the panorama editor window.
