A good strategy is described in the Hugin tutorial titled "Stitching Flat Scanned Images" .
The same strategy seems to work OK using the current version of PTGui (version 4.1). Older versions of PTGui had problems with large shifts (d/e) causing images to be cropped in the panorama editor window.
